Hospital in the Home provides direct patient care to inpatients in their home environment. The service provides community-based care for inpatients in their home from Palm Cove to the north, Gordonvale to the south and the back of Lamb's Range to the west.

About you:
In your new role you will:
Hospital in the Home (HITH) CN is responsible for assessment and support of HITH patients' health literacy.
Act as an advocate for the clients and client groups to ensure they access appropriate and inclusive health care.
Provide leadership to effectively plan, manage and evaluate client care activities at an advanced practitioner level within the Hospital in the Home Service.
