We’re seeking a compassionate, Spanish-speaking Care Partner to support older adults to live independently, access quality care, and thrive in their communities as part of the new Support at Home Program.
About then Role
As a Care Partner, you will support Spanish-speaking older adults to access and navigate aged care services, ensuring they receive personalised, culturally appropriate care. Working closely with clients, families, and service providers, you will coordinate supports that promote independence, wellbeing, and quality of life across Melbourne’s South East and Eastern regions.
Care Partner- Aged Care Services
– Part-time position (4 days per week), with an anticipated progression to full-time as service demand and program growth continue.

Key Responsibilities
- Manage a caseload of participants receiving Support at Home services.
- Complete assessments, care plans, clinical risk reviews and ongoing goal monitoring.
- Coordinate services and referrals with GPs, allied health professionals and community providers.
- Support participants to understand and effectively manage their individual budgets and available services.
- Monitor spending to prevent under or overspending and ensure resources align with changing needs.
- Review care plans following hospitalisations, clinical events or significant changes in circumstances.
- Promote participant choice, independence and informed decision-making.
- Maintain accurate documentation, care notes and reporting.
- Support quality improvement, incident management and continuous improvement initiatives.
- Ensure compliance with the Support at Home Program and Strengthened Aged Care Quality Standards.
About You
You are passionate about supporting older people and understand the importance of culturally safe and appropriate services, person-centred care.
Essential Qualifications & Experience
- Fluent Spanish and English language skills.
- Experience supporting older people or vulnerable clients within aged care, health, NDIS or community services.
- Experience in care planning, assessments, service coordination and clinical case management.
- Experience supporting participants with complex needs.
- Experience managing consumer budgets and service utilisation.
- Understanding of Consumer Directed Care (CDC), Support at Home and Australian aged care reforms.
- Strong computer skills, including advanced proficiency in Microsoft Excel and experience using client management systems.
- Current Victorian Driver Licence and willingness to travel.
Desirable
- Qualification and experience in Nursing, Allied Health, or a related discipline.
- Experience in care planning, care coordination, or case management.
- Knowledge of the aged care sector, including Support at Home (SAH) and CHSP.
Essential
- Qualification in Care Management, Social Work, or a related field.
- Experience supporting older people or vulnerable clients in aged care, health, NDIS or community services.
- Strong communication and relationship-building skills.
- Ability to work in a person-centred and culturally sensitive way.
