Jac’s Place is seeking a female Support Worker to provide ongoing support to a female NDIS participant near to the Forest Lake area. We are looking for someone who is kind, reliable, motivated, and passionate about helping people achieve their personal, social, and study goals. An interest or background in Social Work highly desirable.
Due to participant preferences and support requirements, applications are open to female support workers only.
Support workers who live in or near the Forest Lake area are strongly preferred, as consistency and reliability are highly important to the participant.

About the Role
We are currently seeking a dedicated female Support Worker to provide one-on-one support to a female participant near to the Forest Lake area.
This role is ideal for someone who enjoys building meaningful connections, mentoring young adults and helping people work towards greater independence, confidence, and personal achievement.
This role will involve capacity building and assisting participant in her Tafe studies.
Consistency is essential for this participant. We are seeking someone who can commit to providing regular, ongoing support and develop a strong, trusting relationship over time.
There is also potential for additional hours and opportunities to support other Jac’s Place participants in the future.
Key Responsibilities
- Assisting with daily living tasks, routines, and personal care
- Supporting capacity building and skill development
- Assisting the participant to achieve personal and study-related goals
- Supporting community access, appointments, and social participation
- Encouraging independence, confidence, and goal achievement
- Writing clear and accurate progress notes and incident reports
- Maintaining professional boundaries while delivering person-centered support

About You
You are:

- Strong spoken English skills required to ensure participant safety and understanding
- Passionate about empowering people to achieve their goals
- Professional, empathetic, and person-centred in your approach
Highly Desirable
- Background, qualifications, or current studies in Social Work
- Experience supporting participants to achieve educational, personal, or community engagement goals
- Experience supporting participants with Autism Spectrum Disorder (ASD) and Attention Deficit Hyperactivity Disorder (ADHD)
- Experience supporting individuals with behavioral support needs
Position Requirements
- 100 points of identification (Passport, Driver Licence, Proof of Age Card, etc.)
- Valid Australian working rights (Birth Certificate, Passport, VEVO, etc.)
- Current Driver Licence and access to a reliable vehicle
- NDIS Worker Orientation Module (or willingness to obtain)
- Qualification in Human Services or related field (completed or currently studying)
- Working with Children Check (or willingness to obtain)
- NDIS Worker Screening Clearance (or willingness to obtain)
- Current First Aid and CPR Certificate (or willingness to obtain)
- Comprehensive Car Insurance (or willingness to obtain)
