About the role
In this role, you'll support our clients in their homes and in the community. You will be responsible for various activities including in-home support, personal care, transport, and social & community access.
Responsibilities and duties may include:
- Support clients to participate in age-appropriate activities and tasks
- Provide direct care assistance to each client in accordance with their service plan and as directed.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes as required.
- Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
- Personal care is provided to clients in a caring and supportive way.
- Be available for ~ 20 - 30 hours fortnightly
About you
You must have:
- Certificate III in Individual Support (Home and Community Care, Mental Health or Disability) or equivalent
- A current Provide First Aid Certificate (HLTAID003)
- A current Provide CPR Certificate (HLTAID001)
- A current National Police Clearance (Obtained in the last 12 months)
- Worker’s Screening Check or current Yellow Card
- Blue card
- NDIS Worker orientation (or willing to obtain it)
- Reliable transport
- Experience with disability and AGED CARE services would be an advantage
We offer competitive pay rates. Pay $36-$40 + 12% Superannuation
