Job Overview
Silverchain Group is seeking a compassionate Registered Nurse (Community) to provide high-quality nursing care to clients in their homes and community settings. The role involves delivering clinical care, developing individualized care plans, promoting health and wellbeing, and working with multidisciplinary teams to support positive patient outcomes.
Key Responsibilities
Assess clients and develop individualized nursing care plans.
Administer medications and provide clinical treatments, including wound care and other nursing procedures.
Deliver community-based nursing care and support chronic disease management and rehabilitation.
Educate clients and caregivers on health conditions, medication management, and home safety.
Maintain accurate clinical documentation and ensure compliance with professional standards.
Collaborate with healthcare professionals to coordinate person-centered care.
Requirements
Bachelor of Nursing or equivalent qualification.
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
Strong clinical assessment, communication, and decision-making skills.
Ability to work independently in community settings.
Current Australian driver's licence and willingness to travel.
Experience in community nursing, aged care, or home healthcare is preferred.
