We're seeking a Care Manager to join our Aged Care team on a 12-month maximum-term contract (full-time), supporting a caseload of clients across the Loddon Mallee region, including Bendigo, Echuca and Swan Hill.
This is a primarily work-from-home opportunity, offering flexibility while still allowing you to make a meaningful impact in your local community. You'll attend our Abbotsford head office approximately once per month, with all travel and accommodation expenses covered.

About the role
Reporting to the Team Leader, you will work closely with older Australians and their families to understand their goals, assess their needs, develop personalised care plans and connect them with the services and supports they need to live independently at home.
You will build trusted relationships with clients and stakeholders, provide ongoing advice and guidance, and ensure that services remain responsive to changing needs over time.
Key responsibilities
- Complete client assessments and develop person-centred care plans.
- Support clients to access services that promote independence, choice and wellbeing.
- Monitor client needs and conduct ongoing care plan and budget reviews.
- Build strong relationships with clients, families, service providers and referral partners.
- Collaborate with Service Coordinators to ensure services are delivered safely and effectively.
- Maintain accurate client records and documentation.
- Provide advice and support that helps clients make informed decisions about their care.
- Identify opportunities to improve client outcomes and service delivery.

About you
You'll be passionate about helping people, building relationships and delivering excellent client outcomes.
To be successful, you will have:
- A minimum Diploma qualification in health, social or community services, or be working towards a relevant qualification.
- Strong communication and relationship-building skills.
- Excellent planning, organisational and time management skills.
- A client-focused approach and genuine desire to support older Australians.
- Strong attention to detail and accurate record-keeping skills.
- The ability to manage competing priorities and work autonomously.
- Confidence using Microsoft Office and other business systems.
Highly regarded but not essential:
- Experience in aged care, community services, health, disability or human services.
- Knowledge of Home Care Packages and aged care programs.
- Experience in care management, case management or support coordination.
