Support Worker – Disability Support (Subcontractor)

About the Role
Horizon Support Group is seeking a reliable, compassionate and motivated Disability Support Worker to join our team as a subcontractor. This role involves supporting NDIS participants to achieve their goals, increase independence, and engage in meaningful community activities.
This position offers approximately 25 hours per week, with a flexible roster based around participant needs. Community access hours and shifts may vary depending on participant schedules, appointments and support requirements.
Key Responsibilities
- Provide person-centred supports in line with participant goals and support plans.
- Support participants with community access, appointments and social activities.
- Assist with daily living skills and promote independence.
- Provide personal care where required.
- Administer medication (where trained and authorised).
- Complete accurate progress notes, incident reports and required documentation.
- Follow NDIS Practice Standards, organisational policies and participant behaviour support plans where applicable.
- Maintain professional boundaries while promoting participant choice, dignity and respect.
Contractor Requirements
Applicants must have:
- An active ABN.
- Public Liability Insurance.
- Professional Indemnity Insurance.
- Workers Compensation (if applicable to your business structure).
- Current NDIS Worker Screening Check.
- Current First Aid & CPR Certificate.
- Current Australian Driver Licence.
- Reliable and comprehensively insured vehicle.
- Smartphone for rostering, communication and documentation.

- Comfortable working a roster that may include weekdays, evenings and occasional weekends.

Pay: $40.00 per hour
