About the role
Reporting to the Director, Primary Health Care Improvement (PHCI), you will lead a team to drive system-level improvements across primary care. You'll play a pivotal role in delivering capability-building initiatives that support providers to implement person-centred, evidence-based care aligned with health reform priorities.
This is a hands-on leadership role where you will translate strategy into real-world outcomes, influencing and partnering with primary care providers to strengthen quality of care across our diverse communities.
You will lead the end-to-end delivery of PHCI programs, ensuring strong stakeholder engagement, alignment to strategic priorities, and measurable impact. In parallel,you will build internal and external capability and position NWMPHN as a trusted partner in primary care improvement and system reform . You will apply structured approaches to change management and engagement, support providers to both adapt and sustain new ways of work.
A key focus of the role is leveraging primary care data and insights, including managing general practice data insight and aggregation tools, to monitor performance, identify improvement opportunities and drive continuous quality improvement to improve population health outcomes.

About you
You're a confident, people-focused leader with a track record of driving improvement in primary care or health systems. You're comfortable working in complexity and can translate strategy, policy and data into practical outcomes.
You'll bring:

Relevant qualifications in public health, health sciences or a related field (or equivalent practical experience).

Strong understanding of the Australian primary care system, including general practice, PHNs and the current reform landscape, with the ability to translate policy into practice.

Proven people leadership experience, with the ability to coach, develop and drive performance in a high-performing team.
-
Experience designing and delivering quality improvement and capability-building initiatives, turning complex concepts into practical, measurable outcomes.

Strong data and systems thinking capability, using insights to inform decisions, lead change and drive continuous improvement.
-
Highly developed stakeholder engagement and influencing skills, with the ability to build strategic relationships and drive behaviour change.

Highly developed stakeholder engagement and influencing skills, with experience working across complex environments, including commissioning and/or project delivery.

This role may require offsite travel. A current Driver's License and reliable vehicle are essential.
You'll stand out with:
