About the Opportunity
Make a lasting difference in the lives of children and families by joining the Sustaining NSW Families (SNF) Program at Kurri Kurri Community Health. The SNF program is an evidence-based Sustained Health Home Visiting (SHHV) program that supports families through the transition to parenting, from the early newborn period through to toddlerhood at two years of age. Led by Child and Family Health Nurses and supported by Allied Health professionals, the program uses a strengths-based partnership approach to build trusting and therapeutic relationships with families. Based at Kurri Kurri Community Health, just 25 minutes from Newcastle, this is an established service with more than 16 years of experience supporting local families. You'll join a highly experienced, cohesive and supportive team who are passionate about improving outcomes for children and families.

What You Will Be Doing
As a Child and Family Health Nurse, you will work in partnership with parents and carers to promote infant mental health, optimal child development and positive parenting. You'll provide individualised support to families identified through the Safestart process as experiencing level 2 vulnerabilities, helping them build confidence, strengthen their parenting skills and achieve their goals.
You will:

Provide sustained health home visiting to families.

Develop and maintain therapeutic relationships with families using a strengths-based and partnership approach.

Promote infant mental health, positive parenting and optimal child development.

Provide education and practical support throughout the transition to parenting.

Undertake comprehensive assessments and develop individualised plans of care.

Provide ongoing case management and coordinate care with Allied Health and other health and community services.

Advocate for children and families and support them to access appropriate services.

Monitor family progress and adapt care plans to meet changing needs.

Work collaboratively with the multidisciplinary team to achieve positive outcomes for children and families.

Support families from the newborn period through to their child's second birthday.
About you
Our ideal candidate is a compassionate and experienced Child and Family Health Nurse who is passionate about supporting children and families and wants to make a meaningful difference within the community. You will have strong interpersonal and communication skills and be confident building trusting relationships with families from a range of backgrounds. You will use a strengths-based approach to partner with families, recognising their existing capabilities while supporting them to build confidence and achieve positive outcomes.
Experience as a Child and Family Health Nurse is essential; however, we also welcome nurses with additional transferable experience in areas such as paediatrics, mental health, community nursing or family support, who are passionate about developing their skills in Child and Family Health.
