About the Role
We are seeking an experienced and motivated Residential Care Officer to join our growing team at uCare iCare Pty Ltd, a leading NDIS-registered provider committed to empowering individuals with disabilities to live fulfilling, independent, and meaningful lives.
In this full-time role, you will be responsible for the coordination, documentation, and oversight of participant care within our Supported Independent Living accommodations. You will work collaboratively with participants, families, support staff, and allied health professionals to ensure that each participant’s individual needs and goals are met in line with the NDIS Practice Standards and organisational policies.
Key Responsibilities
· Develop, review, and implement individualised support and behaviour plans with participants and their support networks.
· Conduct intake, assessment, goal planning, progress reviews, and transition planning; keep plans current and actionable.
· Oversee and support Disability Support Workers so daily supports align with participant needs, preferences, and NDIS goals.
· Oversee, and when required deliver, personal care, medication prompts, mealtime management, transport, community access, and household tasks with dignity and choice.
· Monitor wellbeing and provide timely, proportionate responses to behavioural, emotional, or environmental risks; escalate when required.
· Maintain accurate, up-to-date case notes, reports, and participant documentation to privacy and NDIS quality standards; support audits.
· Record progress toward goals and adjust routines or strategies following reviews, incidents, or clinical advice.
· Implement risk controls, complete incident reports, and contribute to continuous improvement activities.
· Communicate routinely with families/guardians, support coordinators, and allied health to coordinate appointments, reviews, and follow-ups.
· Guide shift practice, ensure consistent routines, safe work methods, and quality documentation across the house.
· Plan and support skill-building, recreation, and community engagement aligned to participant interests and goals.
· Participate in team meetings, training, and professional development to maintain currency of practice and compliance.
About You

Essential Requirements
- Certificate IV or Diploma in Disability, Community Services, or equivalent qualification.
- Minimum of 2 years’ experience in case management or residential coordination within the disability sector.
- Current Working with Children Check and NDIS Worker Screening Check.
- Current First Aid and CPR certification.
- International police check for non-citizens.
- Current Australian Driver’s Licence and reliable vehicle.
- Comprehensive Car Insurance.
- Valid and current 100 points ID (including Driver’s licence + Passport / Australian birth certificate + Valid visa for non-citizens).
- NDIS Worker Orientation Module certificate.
- Infection control training within 12-months.
- Proficiency in documentation, planning, and reporting in line with NDIS standards.
- Demonstrated ability to work collaboratively with multidisciplinary teams.
Desirable
- Experience managing or supervising support staff within residential or community settings.
- Experience supporting individuals with complex needs and Behaviour Support Plans.
- Strong understanding of NDIS Quality and Safeguarding Framework.
