About LYZ Food
LYZ Food provides nutritious, safe and convenient ready-to-heat meals for older adults living at home, clients receiving care services, and Asian clients supported by aged care and community service providers.
We are looking for two full-time or part-time Client Intake & Service Coordinators to support referral follow-up, client enquiries, client intake, administration, documentation and invoicing-related tasks.
Key Responsibilities
- Respond to enquiries from clients, families, care finders, case managers, allied health professionals and service providers.
- Manage incoming referrals and clearly explain LYZ Food’s services, ordering process, delivery arrangements, co-payment process and invoicing structure.
- Complete client intake, including client details, provider information, billing contacts, dietary preferences, allergy information and delivery notes.
- Follow up referrals and assist clients to start using the service.
- Maintain accurate client records, referral notes and provider information.
- Assist with order coordination, customer feedback, service follow-up and delivery-related enquiries.
- Assist with invoice preparation, co-payment records, provider billing information and payment follow-up.
- Organise electronic and paper files, including client documents, provider records and administrative files.
- Work with internal teams to ensure smooth service delivery.
Requirements
- Fluent English communication skills, both written and verbal.
- Fluent Mandarin and Cantonese communication skills are essential.
- Experience in customer service, administration, intake, aged care, home care, NDIS, community services or a related field is preferred.
- Confident and professional communication with clients, families, case managers and service providers.
- Good computer skills, including email, spreadsheets and basic document management.
- Strong attention to detail and ability to follow up tasks independently.
