Blue Mountains District ANZAC Memorial Hospital is an acute hospital that supports a population of approximately 80,000 and is the only ANZAC Memorial hospital remaining in NSW. An exciting opportunity exists for an experienced and compassionate Clinical Nurse Specialist to join the Aged Care Services Emergency Team (ASET) and Acute to Aged-Related Care Service (AARCS). This rewarding role is dedicated to improving the health outcomes of older people by providing specialised assessment, care planning, coordination and support across the acute care continuum.
As a Clinical Nurse Specialist within ASET, you will play a key role in the early identification, screening and assessment of older people presenting to the Emergency Department. Working as part of a multidisciplinary team, you will ensure patients aged 70 years and over, and Aboriginal and Torres Strait Islander patients aged 50 years and over, receive timely, coordinated and person-centred care. Your work will contribute to minimising unnecessary hospital admissions, reducing length of stay and supporting safe and effective discharge planning.
With objective of the AARCS role, you will focus on identifying and coordinating discharge support needs for older patients admitted to hospital. You will work closely with patients, families, carers and healthcare professionals to facilitate access to appropriate community services, residential aged care facilities and ongoing support. Collaboration is at the heart of this position, with strong partnerships maintained across hospital teams, social workers, the Aged Care Assessment Team (ACAT) and residential aged care providers to achieve the best possible outcomes for patients.
