Key Responsibilities:
· Undertake in home client clinical care including wound care, catheter changing and medication management.
· Undertake and document relevant assessments that support client on-boarding and ongoing care in alignment with the Aged Care Standards and organisation policies.
· Where required refer and collaborate with other clinicians and health care professionals to facilitate a multi-disciplinary approach to care in order to achieve effective outcomes for clients
· Ensures the Occupational Safety and Health (WA) 1984 Act and policies are complied with at all times.
To be successful in this role you will have the following attributes:
· Hold current registration as a Registered Nurse with AHPRA
· Demonstrated ability and experience in providing education, mentoring and competency assessment for health care team members
· Strong computer skills and experience with client database systems
· Knowledge of OSH legislation and requirements
· Current driver’s license
· Knowledge in Aged Care with experience in home packages and Community Home Care standards highly desirable.

Pay: $50.00 – $53.00 per hour
