Responsibilities:
- Provide high-quality nursing care to patients in the community setting, including assessments, medication administration, wound care, and catheter care.
- Collaborate with interdisciplinary team members, patients, and their families to develop and implement comprehensive care plans that meet patients' individual needs.
- Educate patients and their families about health management, disease prevention, and self-care techniques.
- Monitor patients' condition and response to treatment, and make appropriate adjustments to care plans as needed.
- Maintain accurate and up-to-date documentation of patient assessments, care provided, and patient outcomes.
- Act as a resource and mentor to other team members, providing guidance and support as needed.
- Adhere to all relevant policies, procedures, and standards of practice, including infection control protocols and safety regulations.
Qualifications:
- Current and valid Registered Nurse (RN) and Enroll Nurse Registration .
- Minimum of 6 months of either hospital nursing experience or aged care, community care
- Knowledge of ACFI and aged care quality standards
- Excellent communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary team members.
- Ability to work independently and manage time effectively in a community care setting.
- Strong critical thinking and problem-solving skills to assess patient needs and make appropriate clinical decisions.
- Familiarity with electronic health records (EHR) and other relevant healthcare technology.
- Valid driver's license and reliable transportation for home visits.
- able to travel for work at multiple sites.
