Are You the Right Fit?
Eligibility criteria: To apply, you must:

Identify as Aboriginal and/or Torres Strait Islander.
Be aged 16 years or older.
Be committed to attending TAFE one day per week to complete a Certificate III.
Be available to work 38 hours per week.
No prior experience is required, just a willingness to learn and grow.

We are currently offering an Aboriginal Health Worker Traineeships within our Aboriginal Chronic Care Team. This traineeship offers the opportunity to develop skills within Aboriginal Chronic Care services, supporting Aboriginal patients living with or at risk of chronic health conditions. As a trainee you will work closely with Aboriginal Health Workers and clinical teams to support care coordination, health education, cultural support, and improved access to services, helping to promote better health outcomes for Aboriginal communities.

Selection Criteria
To be considered for this position,please ensure you address the below questions:

Briefly outline why you are interested in this position?
Willingness to learn case management and care coordination for delivering health or social services to Aboriginal communities.
Willingness to establish and maintain partnerships with other staff and government, non-government and community organisations. Including working in community-based settings with Aboriginal communities.
Willingness and capacity to complete a Certificate III in Aboriginal Primarily Health Care throughout the traineeship program.
Applicants must possess an Australian C-Class driver's license (including P1 or P. Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
