About Us
Gentle Care Nursing Services Pty Ltd provides high-quality nursing and support services to clients in the community, including DVA Community Nursing, NDIS participants, aged care clients and private care clients.
We are seeking a passionate and dedicated Community Worker Officer to join our Community Services team. In this role, you will play a key part in supporting individuals and groups facing social, economic, or health-related challenges, helping them access the services and resources they need to improve their wellbeing and independence.
About the Role
As a Community Worker Officer, you will work directly with clients to assess needs, develop tailored support plans, and connect them with relevant community services and government programs. You will also collaborate with partner organisations and multidisciplinary teams to deliver coordinated, client-focused support.
Key Responsibilities
- Conduct client assessments to identify needs, strengths, and barriers
- Develop and implement individualised support plans with clients
- Provide information, referrals, and advocacy to community and government services
- Facilitate group activities, workshops, and community engagement initiatives
- Maintain accurate case notes, records, and reports in line with confidentiality requirements
- Work with internal teams and external agencies to coordinate support services
- Support clients with crisis intervention and conflict resolution where required
- Promote awareness of social issues within the community
- Engage in ongoing professional development and training
About You
- Relevant qualification in Community Services, Social Work, Psychology, or related discipline
- 1-2 years of Experience working with diverse and vulnerable populations
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively in a team
- Knowledge of community resources, government programs, and social policies
- High level of empathy, patience, and cultural sensitivity
- Strong organisational and documentation skills
- Current driver’s licence (if required)
- Valid National Disability Insurance Scheme Worker Check (NDISWC) / Working With Children Check and (WWCC) /or Police Check
