About the Role:
Join Country Health Connect as a NDIS Coordinator and make a meaningful difference by helping people with disability achieve their goals and live more independently. Working within a supportive multidisciplinary team, you'll coordinate person-centred services, build strong relationships with participants and their support networks, and deliver high-quality, tailored support that promotes choice, independence and positive outcomes.
What We're Looking For:
Essential Criteria
Strong communication and relationship-building skills with participants, families, carers and stakeholders.
Ability to work independently and collaboratively, using sound judgement and initiative to achieve outcomes.
Strong organisational, analytical and problem-solving skills, with the ability to manage competing priorities.
Experience working in community and multidisciplinary settings, with proficiency in Microsoft Office applications.
Knowledge of confidentiality principles and person-centred, consumer-directed care.
Desired Criteria
Certificate III or IV in Aged Care, Community Services or Disability Services.
Knowledge of the National Disability Insurance Scheme (NDIS) and financial reporting requirements.
Positive, adaptable attitude with a willingness to learn and embrace change.
