About the role:
As a Community Support Worker at Providence Disability & Ageing Support Services, you will be responsible for providing direct support to individuals with disabilities, facilitating their access to community resources, and promoting their overall well-being. You will work collaboratively with clients, their families, and other service providers to create and implement personalized support plans that align with each individual's goals and aspirations.
Main Duties and Responsibilities:
1. Assess and Develop Support Plans: Conduct individualized assessments to create and implement personalized care plans that promote independence and well-being, tailored to clients' physical, emotional, and social needs in both home and community settings.
2. Provide Practical and Emotional Support: Offer practical support and emotional assistance to individuals with disabilities and their families, including referrals to essential services like housing, medical care, and emergency support.
3. Promote Independent Living: Help clients develop independent living skills such as personal care, social engagement, and community participation to enhance their daily life and autonomy.
4. Advocate for Clients: Advocate for the rights of individuals within the community, ensuring equal access to opportunities in employment, education, and healthcare while promoting inclusivity.
5. Monitor Client Progress: Regularly review and adjust care plans based on clients' progress, ensuring that their evolving needs are met in both home and community settings.
6. Coordinate and Collaborate: Work with families, caregivers, and multidisciplinary professionals to provide integrated care, ensuring effective communication and seamless service delivery.
7. Community Outreach and Education: Lead outreach initiatives, including mobile disability services and workshops on disability rights, employment, and self-advocacy to empower clients and the broader community.
8. Support Mental Health and Crisis Intervention: Assist clients with mental health challenges and recovery planning, providing crisis intervention when needed.
9. Maintain Records: Ensure accurate and up-to-date documentation of client interactions, assessments, and services, adhering to confidentiality standards.
10. Transition Planning: Support clients transitioning from institutional care to community-based living, ensuring a smooth and successful integration.
Requirement:
- Diploma or higher qualification in Disability, Community Services, Social Work, Individual Support or a related field.
- Previous experience working with people with disabilities, community services or related support services is highly desirable.
- Knowledge of the National Disability Insurance Scheme (NDIS), disability rights and relevant legislation.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and collaboratively within a team.
