Clinical Operations Manager – HITH, Community Nursing & Care Services
Location: Melbourne (Hybrid – Coburg North Office and Community-Based)
Employment Type: Full-Time or Part time 4 days per week
Salary: Negotiable based on experience ranging from $65-$75 per hour

Need2Heal is a growing, nurse-led healthcare organisation providing Hospital in the Home (HITH), post-acute care, NDIS, Support at Home (SAH), aged care and private nursing services throughout Melbourne.
We are seeking an experienced Clinical Operations Manager to lead our clinical service delivery, support our growing workforce, and ensure the continued provision of safe, high-quality, patient-centred care across multiple funding streams.
This is a senior leadership role suited to an experienced Registered Nurse with extensive Hospital in the Home experience, strong operational management skills, and a sound understanding of NDIS and Support at Home service delivery.
About the Role
Reporting directly to the Directors, the Clinical Operations Manager will oversee the day-to-day clinical and operational management of Need2Heal's nursing and care services.
The successful candidate will be responsible for workforce management, clinical governance, quality improvement, stakeholder engagement, compliance, and service growth while ensuring exceptional outcomes for our clients and referral partners.
Key ResponsibilitiesClinical Leadership & Governance
- Provide clinical leadership and support to Registered Nurses, Enrolled Nurses and Support Workers
- Ensure services are delivered in accordance with clinical best practice, organisational policies and regulatory requirements
- Support clinical decision-making and escalation of complex client matters
- Lead incident management, investigations and continuous improvement activities
- Assist with the development and review of clinical policies, procedures and governance frameworks
- Participate in quality improvement initiatives and accreditation processes
Operational Management
- Oversee daily service delivery across HITH, NDIS, Support at Home and private services
- Manage workforce capacity, resource allocation and service planning
- Support scheduling and workforce optimisation
- Monitor service performance, KPIs and client outcomes
- Identify opportunities for service growth and operational efficiencies
- Ensure services remain financially sustainable while maintaining quality outcomes
Client & Stakeholder Management
- Build and maintain strong relationships with hospitals, HITH programs, NDIS providers, Support at Home providers, Support Coordinators, GPs and allied health professionals
- Manage referrals and onboarding of complex clients
- Participate in case conferences and multidisciplinary meetings
- Ensure high levels of client and stakeholder satisfaction
Workforce Development
- Support recruitment, onboarding and retention of clinical staff
- Provide mentoring, coaching and performance support
- Identify training needs and facilitate professional development opportunities
- Promote a positive workplace culture and staff engagement
Compliance & Funding Knowledge
- Ensure compliance with:
- AHPRA requirements
- NDIS Practice Standards
- Aged Care Quality Standards
- Support at Home program requirements
- Workplace Health and Safety obligations
- Maintain an understanding of funding models, claiming requirements and service agreements across HITH, NDIS and Support at Home programs
About You
To be successful in this role, you will have:
Essential
- Current AHPRA registration as a Registered Nurse (Division 1)
- Minimum 5 years' experience in Hospital in the Home (HITH), acute care or community nursing
- Demonstrated leadership or management experience
- Strong understanding of community nursing service delivery
- Extensive knowledge of NDIS and Support at Home programs
- Excellent clinical assessment and decision-making skills
- Strong communication, stakeholder engagement and relationship-building abilities
- Experience managing complex clinical clients
- Current Victorian Driver Licence
Highly Desirable
- Experience managing community nursing teams
- Knowledge of aged care funding and care management
- Experience with workforce scheduling systems
- Experience in quality and clinical governance
- Experience working with hospital-funded programs and HITH services
