Role
We are seeking an experienced Community Support Worker (Disability) – Team Leader to join Envisioning Life Supports (ELS) in a full-time position.
This is a hands-on disability support role with additional team leadership responsibilities. The successful applicant will continue providing direct support to participants while assisting with staff mentoring, service coordination, documentation and maintaining high-quality, NDIS-compliant supports.
Key Responsibilities
- Provide direct person-centred support to participants in their homes and community
- Assist with personal care, daily living activities, mobility and community participation
- Support participants to achieve their individual goals and build independence
- Implement individual support plans and positive behaviour support strategies where required
- Assist with medication and manual handling where appropriately trained and authorised
- Monitor participant progress, wellbeing, risks and changing support needs
- Maintain accurate progress notes, incident records and other required documentation
- Liaise with families, carers and relevant allied health professionals
- Provide day-to-day guidance and mentoring to Disability Support Workers
- Assist with staff induction and on-the-job training
- Support staff coordination and continuity of participant care
- Act as a first point of escalation for routine staff and participant matters
- Participate in case reviews and team meetings
- Support compliance with NDIS Practice Standards and ELS policies and procedures
Requirements
- Certificate III or IV in Individual Support (Disability), Community Services, Disability Support, or relevant equivalent qualification
- Minimum 2–3 years of relevant disability or community support experience
- Previous experience mentoring, guiding or supporting other staff is highly regarded
- Strong understanding of person-centred disability support
- Knowledge of the NDIS framework and compliance requirements
- NDIS Worker Screening Check (Clearance)
- Working With Children Check (WWCC), where required
- Current First Aid & CPR Certificate
- Current Driver's Licence
- Strong communication and interpersonal skills
- Good documentation and report-writing skills
- Reliable, professional and able to work both independently and as part of a team
