The Role
In this position, you will work autonomously from your home office and travel up to 100km to support individuals living with disabilities or dementia in their homes, schools, workplaces, and community settings.
Key responsibilities include:
- Conducting functional behavioural assessments (FBA) and developing evidence-based PBS Plans (PBSPs).
- Providing coaching, modelling, and capacity-building support to participants, families, and support networks.
- Monitoring and evaluating outcomes using data, incident reports, and team feedback.
- Collaborating with multidisciplinary teams, including OTs, Speech Pathologists, Psychologists, and Support Coordinators.
- Ensuring strict compliance with the NDIS Commission and Disability Services Act (QLD) regarding restrictive practice authorisations.
- Maintaining accurate documentation and progress notes within Halaxy.
About You
Qualifications & Registrations:
- Tertiary qualification in Psychology, Social Work, Occupational Therapy, Speech Pathology, Nursing, Counselling, or a related field.
- Eligibility for registration or membership with a relevant professional body (e.g., AHPRA, AASW, BSPA).
- Completion of, or progression toward, NDIS Commission PBS Practitioner registration (Core or Proficient level) — or a desire to enter via the entry-level pathway.
Experience & Skills:
- Experience developing and implementing PBS Plans under the NDIS.
- Knowledge of restrictive practice authorisations and human rights frameworks.
- Experience working with individuals with intellectual disabilities, autism, acquired brain injuries, or dementia.
- Excellent interpersonal, de-escalation, and report-writing communication skills.
- Ability to work autonomously while remaining a highly collaborative team player.
Mandatory Requirements:
- NDIS Worker Screening Check (or willingness to obtain).
- Current Working with Children Check (Blue Card).
- NDIS Worker Orientation Module certificate (“Quality, Safety and You”).
- Current driver’s licence and access to a reliable vehicle for travel.
