About the Role:
As a Care Partner, you’ll lead the client journey from their very first enquiry through to ongoing care planning and support. This is your chance to be the heartbeat of our service — ensuring every client receives the respect, empathy, and high-quality care they deserve.
Based in our Birkdale and Springwood Offices, you will work with a local client base throughout the metro south of Brisbane and Redlands, regularly visiting clients in person, allowing you to build meaningful relationships and deliver care that is genuinely tailored to everyone.

What You’ll Be Doing:
- Building trusting relationships with clients to design and manage personalised care plans
- Collaborating with clients, families, caregivers, and health professionals to ensure care plans remain aligned with evolving needs and preferences
- Conducting regular visits both in person and over the phone to monitor care and reassess needs.
- Documenting progress and feedback to continually improve care quality
About you:
We’re looking for someone who’s not only skilled — but kind, organized, and driven to make a difference.

Experience in aged care or working with seniors

Home Care Package and Support at Home experience a big plus!

Excellent communicator with strong admin and time management skills

Tech-savvy with Microsoft Office (Excel, Word, Outlook)

Must hold a valid Australian Driver’s Licence

Cert III in Aged Care (or similar) is a bonus – not essential
