Key Responsibilities
- Provide high-quality, person-centred support to NDIS participants in home and community settings.
- Support participants with daily living activities, community participation, and skill development.
- Implement Behaviour Support Plans (where applicable) and follow individual support plans.
- Build positive, respectful relationships with participants, families, and stakeholders.
- Maintain accurate case notes, incident reports, and documentation.
- Promote participant choice, independence, dignity, and social inclusion.
- Work collaboratively with multidisciplinary teams and external service providers.
Essential Requirements
- Demonstrated experience supporting Aboriginal and Torres Strait Islander participants and communities.
- Experience working with participants who have complex support needs and/or those transitioning back into the community.
- Sound understanding of trauma-informed care and the ability to apply a person-centred approach in all aspects of support.
- Strong communication, interpersonal, and problem-solving skills.
- NDIS Worker Screening Check.
- Working With Children Check (if applicable).
- Current First Aid & CPR Certificate.
- Current Australian Driver's Licence and reliable, insured vehicle.
- Current National Police Check (or willingness to obtain).
- Full Australian working rights.
- Certificate III or IV in Individual Support, Disability, Community Services, or a related qualification.
- Experience supporting participants with psychosocial disability, mental health, or complex behaviours.
- Knowledge of NDIS Practice Standards and participant safeguarding.
