Comfort Home Care is seeking a compassionate, experienced and reliable Registered Nurse (RN) to join our growing team and provide high-quality care to older people in their homes and community.

About the Role
As an Aged Care Registered Nurse with Comfort Home Care, you will provide professional nursing care and support to clients in their homes and community settings.
Your responsibilities may include:
- Conducting nursing assessments and monitoring clients' health and wellbeing.
- Developing, reviewing and implementing care plans in consultation with clients, families and relevant healthcare professionals.
- Providing clinical support and nursing care in accordance with individual care plans.
- Monitoring changes in clients' health and escalating concerns appropriately.
- Supporting medication management and administration within your scope of practice.
- Wound care and monitoring where required.
- Supporting clients with chronic health conditions and complex care needs.
- Liaising with GPs, allied health professionals, hospitals and other healthcare providers.
- Providing health education and promoting healthy lifestyle choices.
- Completing accurate clinical documentation, progress notes and incident reports.
- Supporting care staff with clinical guidance and escalation when required.
- Maintaining client dignity, privacy, confidentiality and choice at all times.
- Ensuring care is delivered in accordance with relevant legislation, professional standards and organisational policies.
- Participating in care reviews and contributing to continuous improvement.
- Identifying and managing potential health and safety risks within the client's home.
- Providing guidance and support to support workers where appropriate.
About You
We are looking for someone who is:
- A Registered Nurse with current AHPRA registration.
- Experienced in aged care, community nursing or home care.
- Compassionate, patient and respectful.
- Confident working independently in clients' homes.
- Strong in clinical assessment and decision-making.
- Able to communicate effectively with clients, families and healthcare professionals.
- Organised and able to manage documentation accurately.
- Reliable, professional and committed to high-quality care.
- Comfortable working with clients from diverse cultural and social backgrounds.
- Able to maintain professional boundaries and client confidentiality.
Essential Requirements
- Current Registered Nurse registration with AHPRA.
- Relevant nursing qualification.
- Current First Aid and CPR.
- Current National Police Check.
- NDIS Worker Screening Check, where applicable to the role.
- Working With Children Check, where applicable.
- Current Australian working rights.
- Current driver's licence.
- Reliable and insured vehicle suitable for community-based work.
- Ability to travel between client locations as required.
Desirable
Experience in any of the following would be highly regarded:
- Residential aged care.
- Community nursing or home care.
- Dementia care.
- Palliative care.
- Chronic disease management.
- Wound care.
- Medication management.
- Complex care.
- Clinical supervision of support workers.
