About the Role
The Community Worker will provide support to individuals and community members to promote their wellbeing, independence, and participation in everyday activities and community programs.
Key Responsibilities:
· Assess clients’ needs, circumstances and goals and develop appropriate individual support plans.
· Provide one-on-one and group support to promote independence, wellbeing and community participation.
· Assist clients to access community, health, disability, education, employment and social services.
· Advocate for clients and assist them to overcome barriers and access appropriate services and resources.
· Develop and maintain relationships with community agencies, health professionals and service providers to facilitate referrals and coordinated support.
· Support clients with daily living, decision-making, social skills and participation in community activities.
· Monitor client progress and support outcomes, review individual needs and maintain accurate case records.
· Work collaboratively with clients, families, carers and relevant professionals to achieve client goals.
· Apply relevant NDIS guidelines and organizational policies when delivering supports to eligible participants.
· Complete required documentation, data entry and reporting, and participate in supervision, training and professional development.
Skills and Experience
· Previous experience in community services, disability support, aged care, or a similar role is desirable.
· Relevant qualification preferred.
· Strong communication and interpersonal skills.
· Compassionate, patient, and respectful approach.
· Ability to work independently and as part of a team.
· Relevant qualification in Community Services, Individual Support, Disability, or a related field is desirable.
