About the Role
The Community Worker will provide support to individuals and community members to promote their wellbeing, independence, and participation in everyday activities and community programs.
Key Responsibilities:
- Assess clients’ needs, circumstances and goals and develop appropriate individual support plans.
- Provide practical, social and emotional support to clients to promote independence and wellbeing.
- Assist clients to access community, health, disability, education, employment and social services.
- Coordinate referrals and maintain effective relationships with community organisations and service providers.
- Advocate for clients and assist them to overcome barriers to services, resources and community participation.
- Support clients with daily living skills, decision-making, social skills and participation in community activities.
- Monitor client progress, review support needs and maintain accurate case notes and client records.
- Work collaboratively with families, carers and relevant professionals to coordinate client support.
- Promote inclusion, dignity, choice and self-determination in accordance with organisational policies and relevant regulations.
- Participate in team meetings, supervision, training and other duties relevant to community support services.
Skills and Experience
· Previous experience in community services, disability support, aged care, or a similar role is desirable.
· Relevant qualification preferred.
· Strong communication and interpersonal skills.
· Compassionate, patient, and respectful approach.
· Ability to work independently and as part of a team.
· Relevant qualification in Community Services, Individual Support, Disability, or a related field is desirable.
