About the opportunity
Footprints is seeking a dedicated Link Worker to join our Social Support Services team, delivering the Social Health Connection Service in conjunction with the Brisbane North Primary Health Network (PHN) across the Moreton Bay North sub-region (Caboolture and Kilcoy). Operating under a social prescribing model, this outreach role focuses on empowering adults (18+) to improve their health and wellbeing by reducing social isolation and loneliness, centering on "what matters to me."
This is a one-year maximum-term contract, with the possibility of extension. The annual salary for this position is approximately $92,279 (under SCHADS Awards) including superannuation, plus salary packaging benefits of up to $18,550.
About you
Ideally you will have a Certificate IV or Diploma in Community Services, Social Work, Human Services (or similar field), be close to graduating or you will bring experience delivering community-based support services. You will also bring the following traits:
- Ability to be respectful, calm, and encouraging towards clients and have patience and understanding of others’ journey, including the ability to set boundaries and be self-aware.
- Ability to show initiative, think critically and make judgments based on sound reason and the information/evidence presented.
- Demonstrated capacity to encourage and support individuals to engage in meaningful activities.
- Good written and verbal communication skills, and computer literacy
You will need
- A relevant Certificate IV or Diploma in relevant field (eg. Community Services, Social Work, Human Services) or significant progress towards such a qualification and demonstrated knowledge/experience of the provision of community-based support services.
- Demonstrated experience and understanding of the provision of support services to people who have diverse needs.
- Current class C driver license & own roadworthy vehicle
- National NDIS worker screening check of current Yellow Card
