The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Are you a passionate and experienced Registered Nurse with a strong interest in wound management and community based care? This rewarding role is worked Monday to Friday, offering an excellent work-life balance while delivering high-quality wound management services to patients in the community.
In this role, you will undertake comprehensive assessments, develop and implement individualised care plans, and evaluate outcomes using evidence-based wound care practices. You will provide clinical expertise and leadership to support community nursing staff and other health professionals in the management of complex wounds, while also educating clients and carers to promote self-management and improved health outcomes.
You will embrace a wellness and reablement approach to care, ensuring services are responsive, flexible and aligned with the Australian Aged Care Quality Standards and National Safety and Quality Health Service Standards. Your ability to build strong professional relationships, provide clinical guidance and contribute to continuous quality improvement will be key to your success.
