Due to continued growth and increasing demand for our services, we are seeking a dedicated Community Worker to join our team and contribute to the delivery of essential community programs.
Role Overview
This full-time Community Worker role involves assessing client needs, developing support programs, and providing ongoing assistance to individuals and families. The position requires strong interpersonal skills and a commitment to improving client outcomes in line with ANZSCO 411711 – Community Worker.
Duties and Responsibilities
· Deliver person-centred assistance to participants and their families by facilitating access to appropriate community programs, welfare supports, and essential services.
· Develop individual support approaches by identifying participant goals, evaluating support requirements, and linking clients with suitable education, training, and capacity-building initiatives.
· Assist participants to access available community resources, including healthcare, accommodation assistance, employment pathways, social inclusion programs, and wellbeing services.
· Encourage social participation and strengthen community engagement by supporting individuals to establish positive relationships and increase independence within the community.
· Respond to participant support needs by coordinating timely referrals to relevant health, allied health, counselling, housing, and crisis intervention services where required.
· Support participants to engage in recreational, cultural, educational, and community-based activities that enhance wellbeing and active lifestyles.
· Coordinate and assist with community outreach and support initiatives, including the delivery of essential supplies and participation in community-based programs.
· Work collaboratively with participants, families, carers, and external service providers to implement recovery-focused supports, achieve individual goals, and improve access to mental health and wellbeing services.
Qualifications and Experience
· Relevant qualification at Diploma level or Bachelor in Community Services, Social Work, or a related field.
· Demonstrated experience supporting people with disability, mental health conditions, or complex support needs within community or NDIS environments.
· Sound understanding of person-centred care principles, participant dignity, and community inclusion practices.
· Effective communication, interpersonal, and stakeholder engagement skills.

Pay: From $77,000.00 per year
