About the role
The Connected Care Program aims to deliver better health outcomes and reduce hospitalisation rates by improved integration of care with existing GPs, providing specialist assessment, negotiated, comprehensive care plans and early escalation pathways for clients. The service provides a targeted and individualised service to consumers with complex chronic health or psychosocial needs at risk of recurrent ED presentations, admissions and readmissions. The service is primarily delivered in the home, with telehealth, phone or in-clinic appointments at Barwon Health facilities as required.
The Barwon Health Connected Care Community Clinician will contribute to, and be part of the community-based, multidisciplinary team to support consumers and carers to achieve an optimum level of health independence through:
- Comprehensive, co-ordinated and integrated multidisciplinary services
- Provision of consumer–centred care to community members from a broad range of backgrounds
- Contribute to the development and delivery of high-quality, cost-effective community-based Care Coordination services
- Contribute to meeting the program key performance indicators (KPI’s)
- Deliver evidence-based care coordination aligned with the Wagner Chronic Care Model and organisational guidelines, ensuring seamless transitions across services and effective collaboration with internal and external stakeholders to support consumer-centred care.
- Partner with consumers, families, and carers to develop and implement goal-focused care plans, ensuring care is respectful, culturally appropriate, and aligned with best practice principles including AIDET communication standards.
- Maintain accurate, timely documentation and data entry in accordance with organisational and legislative requirements, participate in quality improvement activities, and ensure compliance with NSQHS standards, information management, and safety frameworks.
- Actively contribute to multidisciplinary team functions, meetings, and service initiatives; support junior staff and students; and maintain ongoing professional development, registration requirements, and adherence to organisational values, policies, and OHS standards.
- Demonstrated extensive clinical experience and knowledge in a relevant health field, including understanding of complex client needs and factors contributing to hospital admissions, particularly in chronic disease contexts.
- Proven ability to work effectively within interprofessional teams, contributing positively to team culture, shared decision-making, and coordinated service delivery.
- Demonstrated experience in undertaking holistic assessments and developing collaborative, client-centred care plans, including the application of self-management approaches.
- Advanced written and verbal communication skills, with demonstrated proficiency in Microsoft applications and clinical databases, and the ability to apply initiative and a systematic approach to problem-solving.
