About the Team
CHQ Pharmacy is dedicated to providing and supporting high quality clinical and medication management services to the children and young people of CHQ. The pharmacy service has a staff of approximately 110, including pharmacists, pharmacy technicians, administrative staff and support staff. The pharmacy team provides services to inpatients, outpatients, hospital in the home, cancer care services, poisons information, aseptic compounding, clinical trials along with safety, quality and governance. The Queensland Poisons Information Service is part of CHQ Pharmacy and provides 24 hour a day emergency service in cases of poisoning.
The Paediatric Palliative Care Service (PPCS) is a state-wide multi-disciplinary service providing consultancy services directly to patients, primary care teams and communities of care within the patient's local community. The purpose of the Paediatric Palliative Care Service (PPCS) is to provide holistic care and support to all patients with life limiting conditions and their families. This is achieved through the provision of competent and compassionate care in an appropriate setting for all children engaged with our service. The service makes good use of resources and looks for opportunities to work within established care pathways without replicating resources and carrying inefficiencies.
About the Role
The position of Pharmacist Senior - Palliative Care will:
Provide high level clinical pharmacy services across the continuum of care to children and their families within the CHQ Paediatric Palliative Care Service including high level participation in multidisciplinary teams (primarily telehealth) related to this sub-speciality.
As part of an inter-professional telehealth / outreach service delivery team, co-ordinate, assist and deliver high level, evidence based, tertiary clinical pharmacy services to patients across the continuum of care within the paediatric palliative care service
Work with service partners both internal and external to ensure that patients referred to the service and their families living outside the south-east corner of Queensland receive optimal and integrated care.
Actively participate in teaching and training, service development and improvement, supervision and departmental planning.
