Casual Registered Nurse, In Home & Community Care

Elouera Home Support is looking for a compassionate and confident Registered Nurse to join our team and provide quality nursing care to clients in their own homes and within the community.
This role is ideal for an experienced nurse who values flexibility, independence and meaningful one on one care. You will have the opportunity to build genuine connections with clients while supporting them to remain safe, healthy and independent at home.
Casual shifts are available with a 2 hour minimum engagement in accordance with the Nurses Award.
About the Role
As a Registered Nurse with Elouera Home Support, you will provide personalised nursing support based on each client's individual needs. The role involves working independently in the community while collaborating with families, support networks and other health professionals to achieve the best possible outcomes.
Your responsibilities may include:
- Wound care and ongoing wound assessment
- Catheter care and management
- Nursing assessments and care plan reviews
- Medication management and monitoring
- Developing and updating individualised care plans
- Identifying clients' physical, mental and psychosocial needs
- Maintaining clear and accurate clinical documentation
- Communicating with clients, families, doctors, case managers and hospital staff
- Supporting clients with activities of daily living where required
- Completing handover notes and relevant clinical forms
- Advocating for clients and supporting positive health outcomes
- Working collaboratively with multidisciplinary health professionals when required
What We Are Looking For
We are looking for a nurse who is caring, reliable and comfortable working independently in clients' homes.
You will ideally have:
- A Degree in Nursing with current AHPRA registration
- A current National Police Check completed within the last 3 years
- A current Working with Children Check
- Current First Aid and CPR certification
- A current driver's licence and ability to travel throughout the area as required
- A good understanding of the health, disability and aged care sectors
- Strong computer and documentation skills
- Good clinical judgement and the ability to work autonomously
- Strong communication and interpersonal skills
- Empathy and respect when supporting older people and people living with disability
- Strong assessment and care planning skills
- Good time management and the ability to prioritise competing needs
- Knowledge of local community and health services
- A commitment to following relevant clinical policies, procedures and professional standards
