The Clinical Partner ( Home Care Registered Nurse) works within HammondCare’s Mission in Action to deliver high-quality, person-centred clinical care that supports clients to live well in their own home.
The role provides evidence-based, best practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed. This role is focused on assessing clinical care needs, developing, and reviewing care plans and delivering excellent clinical care in partnership with clients and key stakeholders.
The role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care. The clinical partner will be travelling to Balliina to Tweeds Heads /Gold Coast but will be required to cover leave and attend to urgent referrals in Lismore, Kyogle and Casino if the other staff are on leave .
