About the Role:
We are looking for a Wellbeing and Health In-reach Nurse (WHIN) Coordinator CNS2 to provide high-quality service to identified local schools, and apply their specialist clinical nursing knowledge, experience and skill in providing complex nursing care for school aged children and young people in these schools.
As the WHIN Coordinator (Clinical Nurse Specialist Grade , you'll provide specialist nursing care and care coordination for school-aged children and young people across identified schools in the West Wyalong and Lake Cargelligo region (Barmedman Public, Ungarie Central, Naradhan Public, Tallimba Public, Weethalle Public, West Wyalong Public and high and Lake Cargelligo Central School) . Based at West Wyalong High School and the West Wyalong Community Health , you'll regularly visit schools throughout the region, delivering both face-to-face and virtual care. Working closely with students, families, school staff and multidisciplinary health professionals, you'll use your advanced clinical knowledge to provide evidence-based, holistic care, improve access to health services and support positive health, wellbeing and educational outcomes for young people.
About You:
Our ideal candidate will demonstrate:

Post-graduate qualifications relevant to the specialty, and at least 3 years full-time equivalent clinical experience in the specialty of the specified post-graduate qualification.

Registered with the Nursing and Midwifery Board of Australia (NMBA), with demonstrated ability to meet practice standards, codes, guidelines and frameworks commensurate with the position.

Demonstrated effective communication including verbal, written and interpersonal skills and information technology skills to provide complex specialty services and influence innovative nursing/midwifery practice.

Demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.

Class C License and live within West Wyalong / Lake Cargelligo Region.
Key Responsibilities:

Work with local school principals, school wellbeing teams, school counsellors and psychologists, school staff and local services to deliver the WHIN coordinator model of care service

Build trusting relationships with students and families to identify health needs and address barriers to accessing services

Establish a family focused approach to connecting students and families to services
-
Identify and triage the health needs of students and families

Assist in coordination of appropriate early intervention, assessments, and referral to services and programs

Contribute to care coordination and case management of vulnerable children, young people and families

Support access to health promotion programs and services to improve the health and wellbeing of the school community and provide support for students and families to improve health literacy and access to services

Work within locally agreed protocols and governance arrangements across Health and Education sectors

Build and maintain relationships with service providers and establish effective referral pathways

Collect and report on service events as required, maintain confidentiality and accurate health care documentation
