Tamworth Aboriginal Medical Service – Aboriginal Corporation (TAMS-AC) is seeking a passionate and community focused ITC Coordinator/Outreach Worker Liverpool Plains to join our team.
This role combines care coordinator, client outreach, advocacy, and practical support to help Aboriginal people living with chronic and complex health conditions access culturally safe, coordinated healthcare. Working within the Integrated Team Care (ITC) Program, you will support clients to navigate the health system, strengthen self management skills, and improve their overall health and wellbeing.

About the Role
As the ITC Coordinator/Outreach Worker Liverpool Plains, you will coordinate and deliver support services for Aboriginal clients living with chronic and complex health conditions. You will work closely with clients, families, community members, GPs, specialists, allied health providers and other health services to ensure clients receive the care they need when they need it.
This is a diverse role that combines care coordination, outreach, client advocacy, health education, and program administration.
Key Responsibilities
Coordinate care and support for clients living with chronic and complex health conditions.
Assess ITC eligibility and develop, monitor and review care plans.
Coordinate referrals, appointments, transport and access to health and community services.
Provide outreach, advocacy and practical support to clients, families and carers.
Help clients understand and manage their health and navigate healthcare systems.
Build strong relationships with community, service providers and key stakeholders.
Maintain accurate records and meet ITC reporting and compliance requirements.
Deliver culturally safe, strengths based care that support Closing the Gap outcomes.
Essential Criteria
Qualification as an Aboriginal Health Practitioner, Registered Nurse, Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice, or relevant experience in health, community services, care coordination or chronic disease management.
Experience working with Aboriginal communities and a strong understanding of culturally safe practice.
Understanding of chronic disease management and the social determinants of health.
Excellent communication, relationship building and advocacy skills.
Strong organisational and time management skills.
Ability to work independently and as part of a multidisciplinary team.
Current driver’s licence
Current Criminal History Check.
Current Working With Children Check (WWCC).
