Support at Home Care Manager
Location: Pennington (Head Office)

Salary Range: $74,558.12 – $80,255.24 annually
Reports To: Operations Manager / Program Manager
About the Role
Are you passionate about making a meaningful difference in the lives of older Australians? We are seeking a dedicated and person-centred Support at Home Care Manager to join our team at our Pennington Head Office.
In this vital role, you will be responsible for coordinating, managing, and reviewing high-quality services for clients receiving care through the Support at Home program. You will act as the primary point of contact for clients, families, and service providers, ensuring care plans support client independence, wellbeing, and quality of life while remaining fully compliant with aged care standards.
Key Responsibilities
As a Care Manager, your day-to-day duties will be centred around four key pillars:
1. Care Management & Client Support
- Conduct comprehensive client assessments, reassessments, and regular reviews.
- Develop, implement, and evaluate tailored, person-centred care plans aligned with individual client goals.
- Maintain regular contact with clients and their support networks to monitor wellbeing and service effectiveness.
- Advocate for client choice, dignity, and independence, assisting them in navigating aged care funding and community resources.
2. Service Coordination & Budgeting
- Manage an allocated caseload efficiently, ensuring services are delivered within approved funding parameters.
- Monitor client budgets and service utilisation to ensure sustainable, high-quality delivery.
- Collaborate closely with external service providers and internal teams to optimise client outcomes.
3. Quality Assurance & Compliance
- Ensure strict adherence to Support at Home Program requirements, Aged Care Quality Standards, and organisational policies.
- Maintain accurate, comprehensive, and up-to-date client records and case notes within our management systems.
- Identify and manage risks relating to client safety; proactively report incidents, complaints, or safeguarding concerns.
- Actively participate in audits, quality reviews, and continuous improvement initiatives.
4. Stakeholder Engagement & Performance
- Build strong, productive relationships with clients, families, healthcare professionals, and key stakeholders.
- Meet or exceed individual and organisational Key Performance Indicators (KPIs).
- Stay informed on aged care reforms, funding models, and industry best practices.

Qualifications & Requirements
- Education: Diploma or higher qualification in Community Services, Nursing, Allied Health, or a related field.
- Compliance Checks:
- Current National Police Check
- Valid Working with Children Check (WWCC)
- NDIS Worker Screening Check
- Aged Care Sector Employment Check
- Certifications: Current First Aid and CPR certificates.
- Licence: A valid, full Australian Driver’s Licence.
Experience & Skills
- Proven Experience: Minimum 12 months of experience in a Care/Case Manager, Care Coordinator, or similar role within aged care, community services, or healthcare.
- Aged Care Knowledge: Sound understanding of the Australian aged care system, including the Support at Home Program and Aged Care Quality Standards.
- Case Management: Demonstrated experience conducting assessments, care planning, and monitoring client funding/utilisation.
- Communication: Highly developed verbal and written communication skills, including report writing and case conference facilitation.
- Systems Literacy: Proficiency in Microsoft Office applications and digital client management systems.
- Mindset: Strong problem-solving abilities, excellent time management, and the capacity to work both independently and collaboratively within a multidisciplinary team.
