The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Are you passionate about improving health outcomes for Aboriginal children and young people and making a meaningful difference in your community? We are seeking an Aboriginal Health Worker to join the Out-of-Home Care Health Pathway Program (OOHC HPP) multidisciplinary team.
In this rewarding role, you will provide culturally responsive assessment, engagement and care coordination for children and young people in Out-of-Home Care. Working closely with health professionals, carers, families and partner agencies, you will help ensure cultural identity, safety and individual needs are recognised, respected and embedded throughout health assessment, care planning and service delivery.
The Aboriginal Health Worker plays a vital role in reducing health inequities experienced by children and young people in care, particularly Aboriginal children and young people who may have experienced trauma, abuse, violence or neglect. Through advocacy, relationship building and culturally safe practice, you will support improved access to health services and help navigate a coordinated pathway through the healthcare system, contributing to better health and wellbeing outcomes.
We are looking for someone who is passionate about Aboriginal health, committed to culturally safe care and able to build strong connections with children, young people, carers, communities and stakeholders. This is a temporary position with the possibility of extension, offering an opportunity to continue making a positive impact within this important program.
