We are currently seeking an experienced and motivated Service Coordinator to join our team and play a key role in coordinating participant supports, ensuring quality service delivery, and achieving positive outcomes for the people we support.
About the Role
As a Service Coordinator, you will be responsible for coordinating services for NDIS participants, ensuring supports are delivered effectively, maintaining compliance, and building strong relationships with participants, families, support workers, and stakeholders.
You will act as a key point of contact, ensuring services align with participant goals, NDIS standards, and organisational policies while contributing to a positive and efficient team environment.
Key Responsibilities
Your duties may include, but are not limited to:
- Coordinating services for an allocated portfolio of NDIS participants
- Ensuring supports are tailored to participant goals, needs, and preferences
- Acting as the main point of contact for participants, families, guardians, and stakeholders
- Building and maintaining positive, professional relationships with participants and staff
- Managing support worker rosters and shift allocations effectively
- Conducting participant intake, onboarding, and service commencement processes
- Developing, reviewing, and updating support plans and service agreements
- Conducting regular participant check-ins and wellbeing reviews
- Managing incidents, service-related concerns, and participant enquiries professionally
- Preparing service quotes and monitoring support budgets in line with NDIS funding
- Maintaining accurate participant records, case notes, and documentation
- Ensuring compliance with NDIS Practice Standards, company policies, and procedures
- Collaborating with internal teams to improve operational efficiency and participant outcomes
- Assisting with reporting, audits, and quality assurance processes
- Supporting the continuous improvement of service delivery and participant experiences
About You
To be successful in this role, you will have:
- Previous experience in a Service Coordinator, Team Leader, Case Manager, or similar role within the NDIS, disability, community services, or aged care sector
- Strong understanding of the NDIS framework and participant-centred approach
- Excellent communication and interpersonal skills
- Strong organisational and time management abilities
- Ability to manage multiple priorities in a fast-paced environment
- Experience with rostering systems, client management systems, and Microsoft Office
- Strong problem-solving skills and ability to handle challenging situations professionally
- A compassionate, proactive, and team-oriented attitude
- Ability to build and maintain positive relationships with participants, families, and stakeholders
