About the Role
As a Care Partner, you play a critical role in supporting clients to live their best life at home. You will conduct in-home assessments, build meaningful relationships, and work collaboratively with clients and their families to design and manage personalised care plans that evolve as needs change.
This is a dynamic, hands-on role that blends problem-solving, advocacy, and compassion. You will be empowered to make decisions, work autonomously, and partner closely with a supportive team that shares your commitment to person-centred care.

What You’ll Be Doing
- Conduct comprehensive in-home assessments and ongoing monitoring of client needs.
- Build tailored care plans that support independence, safety, and quality of life.
- Oversee and manage Support at Home and Support at Home packages within budget.
- Maintain strong, trusted relationships with clients, families, caregivers, and stakeholders.
- Coordinate with internal teams to optimise service delivery and outcomes.
- Ensure compliance with Aged Care Quality Standards and regulatory requirements.
- Identify opportunities to improve services and support business growth.
- Be a calm, confident problem-solver in fast-paced and evolving situations.
What You’ll Bring
- A genuinely person-centred, relationship-driven approach to care
- Experience in a Care Partner, Care Manager, or Case Management role (Support at Home / Aged Care preferred)
- Understanding of Support at Home initiatives (highly regarded)
- Strong communication skills and the ability to empower client choice and participation.
- Confidence working autonomously while contributing positively to a team.
- Proven ability in problem-solving, conflict resolution, and decision-making
Qualifications & Experience
- Diploma level or above in Community, Aged Care/Disability, Nursing & Health, or a related discipline (desirable)
- Minimum 1 year experience in Support at Home Packages or Community Home Support case management
