About the role
The Supported Lifestyle Assistant plays a vital role in working alongside the Supported Lifestyle Lead to deliver a dynamic and engaging program for individuals on their cognitive journey. This position offers creative freedom and the opportunity to contribute to a program that values each individual's physical, social, and cultural background. It is a rewarding role that allows you to make a meaningful impact on people’s lives, fostering connection, engagement, and well-being.
Availability required for Monday to Friday, 8:30am - 4:30pm, permanent full-time position.
Role responsibilities
- Assist in identifying a vast range of activities, both internally and externally, within an agreed-upon budget and Integrate in the monthly Program calendar. Confidently promote the Supported Lifestyle Program among staff and external stakeholders.
- To create positive relationships with the Care and village team to ensure a comprehensive, empathetic, and holistic approach to addressing the social, emotional, and cultural well-being of each resident and escalating to the Care and Management team when residents decline
- To maintain a safe environment by identifying/reporting/rectifying safety risks according to LDK’s policies and procedures.
- To ensure compliance with procedures by keeping up-to-date with all annual mandatory training requirements, such as emergency response and compulsory reporting, recording progress notes according to attendance, participation and behaviour.
- Support residents who are transitioning from independent living to supported living, with 1:1 check-ins and catch-ups, getting to know the residents' history, promoting engagement and trust
Qualifications, Experience and Skills
Certificate 3 in Ageing Support or equivalent
Full Driver’s Licence (Mandatory)
Experience in working within the Aged Care sector & residents with dementia (desirable)
