We are currently seeking an experienced support worker to provide in-home support to an older female client living with advanced dementia. This role is suitable for a calm, patient and reliable worker who has experience supporting clients with high personal care needs, dementia care and continence support.
The client lives at home with family support and requires assistance with daily living due to advanced Alzheimer’s dementia and functional decline. She requires gentle, respectful and consistent support with personal care, continence care, mobility supervision, meal prompting, domestic tasks and companionship.
The successful support worker must be comfortable working in a family home environment and be able to follow care plans, communicate clearly with family members and report any changes or concerns promptly.
The role may include:

Providing respectful personal care support
- Assisting with showering, dressing, grooming and hygiene
- Supporting continence care, including changing continence aids
- Prompting and guiding the client with toileting routines
- Assisting with meal prompting and hydration encouragement
- Supporting safe mobility around the home
- Providing calm companionship and reassurance
- Using a dementia-sensitive approach during all care tasks
- Assisting with light domestic duties, laundry and tidying as required
- Supporting family carers with respite where needed
- Following the client’s care plan and routines
- Monitoring and reporting changes in health, behaviour, appetite, mobility or skin condition
- Maintaining professional boundaries and client dignity at all times
- Completing accurate progress notes after each shift
We are looking for a support worker who is:

Calm, patient and compassionate
- Experienced in supporting people living with dementia
- Confident with personal care and continence support
- Respectful of privacy, dignity and family preferences
- Reliable and punctual
- Able to work independently in a home care setting
- Comfortable following instructions and care plans
- Able to communicate clearly with family members and care coordinators
- Observant and able to report concerns promptly
- Gentle in approach and able to build trust with clients who may not communicate verbally
Applicants must have:

Certificate III in Individual Support, Aged Care, Disability, Community Services, or equivalent experience
- Previous experience in aged care, disability support or home care
- Experience supporting clients with dementia
- Experience with personal care and continence care
- Current National Police Check, or willingness to obtain
- Current First Aid and CPR certificate
- Current driver’s licence and reliable transport
- Full working rights in Australia
- Ability to complete care notes and communicate professionally
