About the Role
We are seeking a qualified and motivated Exercise Physiologist to join our growing allied health team. The successful candidate will provide evidence-based exercise assessment, prescription and intervention for clients in the community, including older adults, people with chronic conditions, mobility limitations, falls risk, deconditioning, and post-hospital recovery needs.
This role involves working closely with clients, families, care managers, support workers and other health professionals to improve client function, independence, strength, balance, mobility and overall quality of life.
Key Responsibilities
· Conduct comprehensive exercise physiology assessments for clients in their homes or community settings.
· Develop individualised, evidence-based exercise programs based on each client’s goals, medical history, physical capacity and care plan.
· Provide interventions to improve strength, balance, mobility, endurance, falls prevention and functional independence.
· Support clients with chronic disease management, reconditioning, rehabilitation and healthy ageing goals.
· Monitor client progress and adjust exercise programs as required.
· Provide education to clients, families and support workers regarding safe exercise, mobility, falls prevention and self-management strategies.
· Work collaboratively with care managers, nurses, physiotherapists, occupational therapists and other allied health professionals.
· Complete accurate and timely clinical documentation, progress notes, reports and recommendations.
· Identify risks, incidents or changes in client condition and escalate appropriately.
· Ensure services are delivered in accordance with relevant aged care standards, company policies and professional guidelines.
· Contribute to continuous improvement, client outcomes and service quality.
Essential Criteria
· Degree qualification in Exercise Physiology or equivalent.
· Current accreditation with Exercise & Sports Science Australia, preferably as an Accredited Exercise Physiologist.
· Experience working with older adults, chronic disease, disability, rehabilitation or community care clients.
· Strong clinical reasoning and ability to provide safe, evidence-based exercise prescription.
· Excellent communication, interpersonal and documentation skills.
· Ability to work independently in community settings and manage a caseload.
· Current National Police Check.
· NDIS Worker Screening Check, if applicable.
· Current First Aid and CPR certificate.
· Valid driver’s licence and access to a reliable vehicle.
· Full working rights in Australia.
Desirable Criteria
· Previous experience in aged care, home care, community health or rehabilitation.
· Knowledge of Support at Home, Home Care Packages, CHSP, NDIS or private allied health services.
· Experience with falls prevention, reconditioning programs, chronic disease management or post-hospital support.
· Fluency in English and Chinese, or English and Korean, is highly desirable.
· Experience using client management systems and electronic progress notes.
