We are seeking a dedicated Social Worker to join our growing team, supporting older adults and their families with high‑quality, person‑centred care.
Key Responsibilities
- Provide assessments, care planning and ongoing case management
- Support clients with navigating My Aged Care, NDIS and community services
- Advocate for client wellbeing, safety and independence
- Work collaboratively with multidisciplinary teams
- Maintain accurate documentation and compliance records
About You
- Degree in Social Work (AASW eligible)
- Strong communication and problem‑solving skills
- Experience in aged care, disability or community services (preferred)
- Empathetic, reliable and committed to client‑centred practice
