About us:
The Transition Care Program provides a quality low intensity allied health intervention, nursing, and case management service, to community-based clients post-discharge from acute and sub-acute hospital settings. Clients will have been recently assessed as eligible for residential care and appropriate for Transition Care (by Aged Care Assessment Team (ACAT)/Intake officers for Transition Care) and as having potential to remain in their community with the appropriate supports, potential to prevent early entry to residential care facilities, or potential to achieve a lower level of care through the delivery of the Transition Care service.
Workloads for this position may be based in areas including the Tablelands regions, Cairns city and surrounding suburbs, Innisfail to the Southern boundary of the CHHHS or Gordonvale hospital. The Department values a well-trained workforce with opportunities for participation in a range of professional support and development activities. The Transition Care team and Older Persons Evaluation and Rehabilitation Assessment (OPERA) service operate as a 'virtual team' across the care continuum.
This position is based out of Cairns/Gordonvale and works closely with the Physiotherapy Department at Cairns Hospital. The role provides services for clients in the Cairns, Cassowary Coast and Tablelands regions and includes travel within work hours to these areas.

About your opportunity:
In this position you will...
Participate in the identification, develop and implementation of quality and service improvement activities that enhance the delivery of the Transition Care Program.
Work cooperatively and effectively in a professional manner with multidisciplinary team members, internal and external agencies, clients, families/carers to achieve a client centred approach.
Deliver evidence based, culturally appropriate physiotherapy clinical assessment, diagnosis, treatment, and case management to the frail elderly population, working in collaboration with a multi-disciplinary team in accordance with prescribed professional and ethical standards utilising a holistic approach to client care.
