Job Description
In this role you will:

Support NDIS participants, along with their families and carers, to identify their needs through assessment and person-centred planning
-
Owning the assessment, development and delivery of evidence-based support plans within a complex support environment
-
Driving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices

Ensuring plans reflect each participant’s goals, strengths and what matters most to them

Building the capability, confidence and understanding of those supporting participants in their day-to-day lives

Working collaboratively within a multidisciplinary team, including mentoring less experienced practitioners

Qualifications
You will need:
-
NDIS Registration as a Behaviour Support Practitioner
-
Tertiary qualifications in a relevant allied health discipline or a related area
-
Current driver's license and own reliable vehicle
-
A genuine passion for working in the disability sector
