Based at Mona Vale Hospital you will be assessing and treating Hospital in the Home (HITH) patients in the NSLHD community. As an independent practitioner you will work in a multidisciplinary team who provides holistic care to acute patients who are safe to treat in their home. This may be face to face in the community or in one of our clinics or via the HealthDirect virtual platform.

What you'll be doing
Based at Mona Vale Hospital, you will assess, plan, implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individual/s and the greater health care team to achieve quality patient outcomes.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) and relevant nursing experience.
- Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.
- Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.
- Sound experience using electronic medical record systems and office applications, and evidence of personal time management capabilities and effective negotiation skills.
- Current NSW Drivers Licence.
