Role purpose:
The Dementia Support Group Facilitator is responsible for facilitating group sessions for individuals living with dementia and supporting their carers and families during program activities. The role assists in creating a safe, welcoming, and engaging environment, delivering planned activities, supporting participant wellbeing, and contributing to the smooth day-to-day operation of the group under the direction of management. The facilitator will work closely with other staff and volunteers to ensure the effective delivery of the program.
Key Responsibilities:
1. Group Facilitation:
- Facilitate and organise dementia support group sessions in accordance with approved program plans.
- Assist with preparing and delivering structured activities suited to the cognitive abilities and interests of participants.
- Encourage participant involvement, social interaction, and inclusion during sessions.
- Foster a supportive and inclusive group environment that encourages participation and interaction.
2. Participant Support:
- Provide practical support, reassurance, and general guidance to participants and their carers during sessions.
- Respond to concerns arising during sessions and refer complex matters to management.
- Encourage and assist participants in maintaining their cognitive and physical abilities.
- Observe participant wellbeing and report concerns to the relevant supervisor.
3. Team Support:
- Volunteers: Work cooperatively with volunteers and provide day-to-day guidance to volunteers regarding tasks during sessions as directed
- Staff: Oversee the daily operations, including facilitators and assistants.
- Chef/Cook: Liaise with catering staff or kitchen staff regarding meal service requirements for sessions
- Support smooth daily operation of the group and report operational issues to management.
4. Coordination and Communication:
- Communicate regularly with the ADS Business Development Manager about group progress and participant needs.
- Collaborate with other staff, volunteers, and external partners to enhance the support group experience.
- Ensure accurate record-keeping and documentation of participant attendance and activities.
- Complete required documentation, review the day's activities, and provide feedback for future sessions after each group meeting.
- Assist with follow-up communication with participants and their families as directed.
5. Resource and Funding program Support:
- Develop and promoting educational materials and resources to participants and their caregivers.
- Support to gather information or materials to apply for funding opportunities to support the ongoing operation and expansion of the dementia support groups.
- Manage resources effectively to ensure the sustainability and quality of the groups.
6. Community Engagement:
- Assist with promoting the dementia support group within the community to attract new participants.
- Support community engagement activities to increase awareness of available services.
- Participate in networking opportunities with local organisations and service providers as requested.
7. General Duties:
- Follow organisational policies, procedures, health and safety, and confidentiality requirements.
- Participate in meetings, training, and continuous improvement activities.
- Undertake other duties within the scope of the role as directed.

Who you are:
Essentials:
- Certificate or Qualification in Aged Care, Community Services, Social work or relevant certification
- Demonstrated caring and understanding of the needs of Vietnamese-aged persons, people with disabilities, and their carers
- Experience working with individuals with dementia and facilitating support groups is highly desirable.
- Strong organisational and leadership skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage multiple priorities
- Police Check clearance
- No ACQSC Banning order
