Supporting patients recovering from elective orthopaedic surgery and acute hospital admissions, the program promotes earlier discharge, faster recovery and greater independence by enabling patients to rehabilitate safely in the comfort of their own homes.

In this role, you'll deliver high-quality rehabilitation services to patients recovering from elective orthopaedic surgery and acute hospital admissions, supporting safe early discharge and improved recovery outcomes.
Using your clinical expertise alongside virtual care and digital health technologies, you'll provide evidence-based, personalised rehabilitation that places patients and their goals at the centre of care.
Key Responsibilities
Deliver comprehensive physiotherapy assessments within patients' homes.
Provide evidence-based rehabilitation programs that support recovery, mobility and independence.
Support patients recovering from elective orthopaedic surgery and acute hospital admissions.
Develop personalised treatment plans tailored to individual goals and outcomes.
Utilise virtual care and digital health technologies to enhance patient engagement and continuity of care.
Collaborate with medical, nursing and allied health professionals to optimise patient outcomes.
Support early discharge and hospital avoidance initiatives.
Maintain accurate clinical documentation and reporting.
Contribute to quality improvement and service development activities.
Champion patient-centred, evidence-based rehabilitation practice.
About You
You're an experienced Physiotherapist who thrives in an autonomous environment, enjoys building meaningful patient relationships and is passionate about achieving exceptional rehabilitation outcomes.
You bring clinical excellence, sound judgement and a commitment to helping people recover safely and confidently at home.
You will bring:
Current registration as a Physiotherapist with AHPRA.
Minimum three (3) years' post-graduate physiotherapy experience.
Experience in rehabilitation, orthopaedics, subacute care or community-based services (highly regarded).
Strong clinical reasoning and evidence-based decision-making skills.
Excellent communication, organisation and time management skills.
Confidence working independently in community settings.
Experience with digital health platforms, telehealth or virtual care models, or a willingness to embrace new technologies.
Current Australian Driver's Licence.
Access to a reliable motor vehicle.
