The Role
Join an incredible team of caring professionals supporting people as they continue their recovery journey after residential rehabilitation. Help clients rebuild their confidence, reconnect with family and community, access ongoing support services, and develop the skills needed to maintain long-term recovery and independence.
As a AOD Community Care Worker, you'll play a vital role in supporting clients as they transition from residential rehabilitation back into the community. You'll provide ongoing outreach support, strengthen recovery goals, assist clients to navigate everyday challenges, and connect them with the services and resources they need to achieve sustainable, long-term recovery.
In this role, you will use your qualifications, experience and professional networks to build strong relationships with health professionals and community organisations, acting as an accessible and trusted representative of our service. You will collaborate closely with government and non-government agencies, mental health and legal services, housing providers and other support networks to coordinate holistic, wrap-around care that promotes long-term recovery and positive client outcomes.
This position requires staff to work in compliance with the Human Services Quality Framework and the National Quality Framework for Alcohol and other Drugs. Staff are also required to adhere to the Society’s Community Services Policy Framework and comply with all SVdP policies and procedures.
This is a Permanent, Part-time position (22.8 hours per week) role based in Mackay.
To be successful in the role, you will meet the following criteria:
- Tertiary qualifications in Social Work, Alcohol & other Drugs, Casework, Mental Health, or similar (Mandatory)
- Experience in Intake and Assessment Processes
- Experience in case plan development and implementation
- Ability to contribute to the development and delivery of individual and group programs to support individuals recovering from drug and alcohol dependencies, as well as their families.
- COVID-19 vaccination status is recommended for this role but not a mandatory requirement.
- Understanding of and commitment to the philosophy of the St Vincent de Paul Society Queensland and an ability to work in accordance with policies and procedures to promote the values and mission of the Society
