Care Manager – NDIS & Aged Care
Hayee Group Service Pty Ltd is seeking an experienced and motivated Care Manager to join our growing NDIS and Aged Care team.
This is an excellent opportunity for a dedicated professional who is passionate about delivering high-quality care, supporting participants and clients, and ensuring compliance with NDIS and Aged Care requirements.

Key Responsibilities
- Care management and service coordination
- Staff scheduling and roster management
- Monitoring service delivery and client outcomes
- Professional email correspondence and documentation
- Complaint handling and resolution
- Incident reporting and management
- Maintaining accurate client records and care documentation
- Liaising with participants, families, support workers, and stakeholders
- Ensuring compliance with NDIS Practice Standards and Aged Care requirements
- Supporting continuous quality improvement initiatives
Skills and Experience
- Experience in care management, service coordination, or a similar role
- Experience in scheduling and workforce rostering
- Strong administration and organisational skills
- Advanced computer skills
- Experience using ShiftCare or similar care management software
- Professional email writing and documentation skills
- Excellent communication and problem-solving abilities
- Ability to work independently and manage competing priorities
- Current driver's licence and own reliable vehicle (preferred)
- Fluent English (Arabic language skills highly desirable)
Knowledge and Understanding
Applicants should have a sound understanding of:
- NDIS Practice Standards
- NDIS Code of Conduct
- Support at Home Program (formerly Home Care Packages)
- Incident Management requirements
- Complaints Management processes
- Privacy and Confidentiality requirements
- Restrictive Practices and Behaviour Support (desirable)
Qualifications
Certificate IV or higher in one of the following:
- Disability Support
- Ageing Support
- Community Services
- Nursing
- Social Work
- Human Services or a related field
Relevant industry experience may be considered in lieu of formal qualifications.
Mandatory Compliance Requirements
Applicants must hold current:

✔ Working With Children Check (WWCC)
✔ First Aid Certificate
✔ CPR Certificate
✔ NDIS Worker Orientation Module Certificate
